Nigeria's Vice President, Yemi Osinbajo's helicopter crash on February 2, 2019 was brought about by an inflight visibility restriction, the Accident Investigation Bureau has uncovered, Sahara Reporters reports.
During the last election campaigns, Osinbajo had left Abuja on a sanctioned helicopter flight to Kabba in continuation of the family visits and next level commitment.
The Agusta Westland 139 chopper with enrollment number 5N-CML possessed and worked by Caverton Helicopters Limited had crash-arrived at Kabba Stadium.
AIB Commissioner, Akin Olateru, said the group of the flight, which took off from the Nnamdi Azikiwe International Airport for Kogi, located the planned landing zone – a football field – and "drew nearer with the speed of 20 kt to around a 100 ft, and entered a drift to land".
Posting the causal elements, Olateru accused the episode for misconception of separation by the group individuals, inappropriate landing techniques and non-adherence to the landing procedure.
He additionally said the helicopter administrator neglected to direct hazard appraisal of the landing readiness.
He stated, "Locally available were 12 people including the Vice President of the Federal Republic of Nigeria, his escort, and three team individuals (Pilot, Co-pilot, and an Engineer).
"The flight team expressed that they located the proposed landing region because of the haze of leftover residue produced by the downwash of a Police helicopter.
"In the wake of locating the football field, the flight group drew nearer with the speed of 20ft to about a 100ft, and entered a drift to land.
"At about 50ft over the ground level, a brownout set in. The flight group lost visual contact with the ground and outside environmental factors.
"The co-pilot started radio elevation callouts "35, 30, 25, 20 and 15". At about 14:34h, the helicopter encountered a hard landing on the correct primary landing gear and turned over onto its correct side. All people on board were cleared healthy.
"The flight team experienced a brownout condition during the drift to land, which prompted the loss of outer visual references, spatial confusion, and loss of situational mindfulness bringing about a misjudgement of separation and ground leeway, as the flight, the group attempted to control the helicopter's developments for landing. The helicopter landed hard and turned over on its correct side.
"Inappropriate landing strategy utilized, non-adherence to organization systems for known or foreseen brownout conditions during landing and absence of hazard appraisal, constrained landing site readiness, and arranging preceding beginning of the flight."
